I-9 Form - Employment Eligibility Verification
Appointments are contingent upon verification of eligibility to work in the United States and receive compensation. The I-9 form requires that you provide evidence of your identity and employment authorization. A list of acceptable documents is located on the last page of the form.
Acceptable ID must be presented on your first day of employment. Appointment paperwork cannot be processed without this verification.
You will need to bring two copies of identification for verification and issuance of ID badge.
Direct Deposit Form
Electronic Direct Deposit of your pay to the bank of your choice is mandatory.
Important Note: The "Check Digit" on the Direct Deposit form is the last digit of the routing number.

Federal Employees Health Benefit (FEHB)
To review and compare plans and premiums, go to https://www.opm.gov/healthcare-insurance/healthcare/plan-information/plans/.
The earliest date that FEHB coverage can be effective is the first day of the pay period following your entry on duty (EOD). See the FEHB Biweekly Effective Date Schedule located under FEHB on the opening page of the JBC's website for a list of Receipt v. Effective dates.
If you are unable to enroll online, contact the JBC directly or fax the SF2809-Health Benefits Election Form to the JBC. The form and fax number are included on the opening page of the JBC's website.

Federal Employees Group Life Insurance (FEGLI)
Enrollment in Basic Life insurance is automatic and begins immediately unless you waive coverage. Basic coverage equals your salary rounded to the nearest thousand, plus two thousand. Optional insurance is also available to you and your family, but you must elect it and be enrolled in Basic.

Flexible Spending Accounts (FSA)
The FSA program gives employees the option to pay for certain health care expenses not covered by insurance as well as dependent care expenses on a pre-tax basis.
Dental and Vision Plans - Enrollment is handled by BENEFEDS.

Federal Employee Dental and/or Vision Insurance Plans (FEDVIP)
Many FEHB plans offer minimal or discounted dental and vision benefits. Employees interested in a comprehensive plan should visit the BENEFEDS website. Dental and vision plans are paid for entirely by the employee.
- DentalBlue is a dental plan that is not available through the FEDVIP program but is offered to federal employees through Blue Cross of Alabama. Blue Cross does require that you have medical coverage with their plan in order to elect supplemental dental benefits and the option (self only or family) is limited to whichever medical plan you elect.
